New ID Cards for employer groups domiciled in Ohio
New combined Medical, Pharmacy and Opti-Vision (where applicable) ID cards for employer groups domiciled in Ohio. The new format includes a 14-digit Alternate ID number which replaces SSN. Other modifications include a Dependent Coverage Yes/No indicator rather than delineating dependents by name. Logo additions and changes were also made as appropriate. Each combination ID card replaces your current medical plan, pharmacy and Opt-Vision ID cards (where applicable). Providers will no longer accept existing ID cards, so please be sure you replace them with the new combination ID card. THERE ARE NO CHANGES IN YOUR EMPLOYEE MEDICAL INSURANCE BENEFITS.
